    Your latest payslip is required. Also attach:

    • Dismissal: the outcome of the disciplinary hearing (note the date you received it).
    • PPN dispute: the PPN certificate and the outcome of the district grievance committee.
    • Misconduct: the charge sheet and any supporting documents.
    • Promotional post: the grievance form and its outcome.

    6 Privacy (POPIA)

    How NATU uses your personal information. The National Teachers' Union (NATU), 47 Biyela Street, Empangeni, 3880, collects the information on this form, including your ID number, Persal number, contact details and the documents you upload, solely to assess and handle your dispute and to represent you.

    Your information is accessible only to authorised NATU legal staff and case managers, and may be shared with your employer, the ELRC, the CCMA, SACE, attorneys or other bodies only where needed to pursue your matter. It is stored securely and kept only as long as required for the matter and NATU's legal obligations.

    You may ask to access or correct your information by contacting NATU Head Office on 035 772 2710. You have the right to lodge a complaint with the Information Regulator.
